New Patient Forms
Before your first appointment, please download all new patient forms and bring them with you to your visit. If you need help filling out any paperwork, our front office staff is happy to assist you. To do this, you can call our office prior to your visit or arrive 20 minutes early to your appointment.
Tip from Dr. Lavin
Sugar-free sports drinks, lemon water or carbonated water does not always correlate to cavity prevention! The acidity (pH) of the beverage can play a vital role in creating an oral environment where cavity-causing bacteria thrive, starting the demineralization process leading to cavities. Everything in moderation is key.

For our patients with insurance, we can provide a complimentary pre-determination of what your insurance may reimburse you toward your treatment. We are also happy to submit claims to your provider on your behalf.
If one of our membership options doesn’t quite fit your needs, we offer 0% APR financing through Cherry Payment Plans.

We are out-of-network for most insurances, but you can still utilize your insurance benefits by exploring your plan’s reimbursement options. We are happy to submit a pre-determination estimate, but cannot guarantee reimbursement. Payment will be expected at the time of your appointment.
