To ensure essential dental services are made accessible to our community, we offer affordable membership plans and 0% APR financing options. We offer single, dual, and family plans that are annual plans that include 100% of preventative care, and if patients require any dental treatment, the fees are discounted.
We do not accept insurance, but we are happy to submit claims on your behalf. See more details below.

New Patient Forms
Before your first appointment, please download all new patient forms and bring them with you to your visit. If you need help filling out any paperwork, our front office staff is happy to assist you. To do this, you can call our office prior to your visit or arrive 20 minutes early to your appointment.
Tip from Dr. Lavin
Sugar-free sports drinks, lemon water or carbonated water does not always correlate to cavity prevention! The acidity (pH) of the beverage can play a vital role in creating an oral environment where cavity-causing bacteria thrive, starting the demineralization process leading to cavities. Everything in moderation is key.
Our insurance approach and financing options
At Lavin Family, Dr. Lavin is committed to higher-quality care tailored to your individual needs, and transparent pricing.
For our patients with insurance, we can provide a complimentary pre-determination of what your insurance may reimburse you toward your treatment. We are also happy to submit claims to your provider on your behalf.
If one of our membership options doesn’t quite fit your needs, we offer 0% APR financing through Cherry Payment Plans and multiple options with CareCredit.
